《上清曲素诀辞箓》，早期上清派经典，约出于东晋南北朝。一卷，收入《正统道藏》正一部。内载六种符箓，即《太极左真人曲素决辞箓》、《上清豁落七元箓》、《上清十天召龙箓》、《上清八威召龙箓》、《上清太玄河图九星箓》、《上清紫纹交带箓》，六种符箓皆出于早期上清派经典，用于召神伏魔，《三洞奉道科戒仪范》卷五已着录以上符箓，故本书当为南北朝上清派道士编纂。
