《上清琼宫灵飞六甲箓》，早期上清派符箓，约出于东晋南朝。《三洞奉道科戒仪范》着录「上清太上琼宫灵飞六甲箓」，即此书。今本一卷，收入《正统道藏》正一部。本书言存思六甲玉女，吞服六甲神符之术。其内容与《上清琼宫灵飞六甲左右上符》一书大体相同，唯文字和符图稍有差异，盖系同一道书之不同版本。
