上阳子金丹大要列仙志，元朝陈致虚撰。原为《金丹大要》之附篇，《正统道藏》本单出为一卷，收入太玄部。本篇列举全真道祖师十六人小传，旨在说明钟吕丹法传承源流。其传法谱系为：东华帝君传钟离权，钟离权传吕洞宾。吕传刘海蟾、王重阳、刘海蟾传董凝阳、张伯端，王重阳传马丹阳等全真七子；马丹阳传黄房公、宋德方；宋德方传李钰、李钰传张模、张模传赵友钦，赵友钦于己巳年（1329）之秋传陈致虚。上述传法谱系，是研究唐宋金元内丹道传承之重要史料。
