太上上清禳灾延寿宝忏，撰人不详。从内容文字看，应为唐宋道士所作。一卷，收入《正统道藏》洞真部威仪类。忏文乃太上所说，用于消灾延寿。内载五十一位上清天尊名号及忏悔文式。宣称凡清信男女，若遇灾病，当于本命日、甲子日、庚申日、三元日、八节日等，以时果香花旙灯供养太上像前，志心归礼天尊，礼诵忏悔文，或请道士代为礼诵忏谢，即可获福延寿。本篇与《太上玉清谢罪登真宝忏》原当为同一组忏文。
