题青元观谭嗣先造。卷前朱绰丁巳岁序称，昔吕先生尝撰《仙公传》1卷，《道藏》之毁有间，贡惟琳仅得閤皂山所记《仙公传》1卷，由惟琳弟子谭嗣先请朱绰为加润色，重加编次。序又称，葛玄成仙“距今盖千二百年”。以此下推，本传重加编次于明。本书赖《道藏》以存。太极葛仙公即三国吴葛玄。传文逐段有注，引《列传》、《抱朴子》、《别传》、《神仙传》、《真诰》、《吴越春秋》、《陶碑》、《上清集》、《王氏神仙传》、《金陵六朝记》、《高道传》和《唐书·艺文志》等古籍。卷末附刘宋方峻《仙公炼丹井铭》和梁陶弘景《吴太极左仙公葛公之碑》及宋崇宁、淳祐封敕。
