全一卷。隋代灌顶（561～632）撰。又作隋天台智者大师别传、天台智者大师别传。系以编年体记述智者大师智顗之行状。收于大正藏第五十册。智顗集天台宗之大成，灌顶则为天台法门之继承者，为智顗门下第一弟子。在智顗之传记中，本书撰著年代最早，记述亦极为详细，故广受重视，与国清百录同为有关智顗行迹之最重要资料。道宣所着续高僧传中之智顗传，即承继本书而来，二者略有出入，可互补不足。关于智顗之资料，另有宋代昙昭之天台智者大师别传注一卷及志磐之佛祖统纪智顗传。又据国清百录序所载，渚宫之法论、会稽之智果等，亦作有智顗之别传。
