全一卷。又作授五戒八戒文。收于大正藏第十八册。一般于受戒之际朗诵本文。内容分为八戒文、五戒文两部分。(一)八戒文，分为五品：(1)稽首请证明品，(2)洗忏往业品，(3)得戒三归品，(4)问持说品，(5)回向发愿品。八戒之项目见于第四品。(二)五戒文，亦分五章：(1)发诚启请章，(2)悔过洗罪章，(3)转邪归敬章，(4)说相合（令）修章，(5)回向愿海章。五戒之项目见于第四章。或谓本戒文为唐代不空（705～774）之译作。
