全一卷。唐代菩提流志译于景龙三年（709）。为密教用以消灾祈福之经典之一。收于大正藏第二十册。本经共有十品，内容叙述佛应观自在菩萨之请而宣说根本陀罗尼、大心陀罗尼、小心陀罗尼，并说诵持此等陀罗尼之种种功德，及其受持方法。本经之异译本有三：(一)实叉难陀所译之观世音菩萨秘密藏如意轮陀罗尼神咒经，(二) 宝思惟所译之观世音菩萨如意摩尼陀罗尼经，(三)义净所译之观自在菩萨如意心陀罗尼咒经。此三种异本与本经皆译于唐代，其中以菩提流志所译者为最完备。
