全一卷。唐代输婆迦罗译。收于大正藏第二十册。内容述说佛在佉罗提耶山时，地藏菩萨为利益一切众生而欲说神咒，经佛允许后，遂说此仪轨；初说大、中、小之三种神咒，次说画像法，其次说印咒、普供养印、总说总印、请赞印，最后说成就十七种护摩之法。
